A typical injury matter can involve an emergency room, an ambulance company, imaging centers, specialists, therapy and pharmacies. Records requests stall when nobody knows who handles them.
For each provider, keep
- The name and the treating doctor.
- The records contact: phone, fax, email or portal, and any outside copy service.
- The billing contact, which is often different.
- The first and last dates of treatment.
- Notes on what was requested and when.
Ask the client early
Clients often forget a provider. Ask about pharmacies, urgent care, chiropractors and any visit before the incident for the same body part, as counsel directs.
